Firearms will ship directly from the manufacturer or distributor to the FFL holder of your choice. Participants are required to submit a legible, current ATF-8 form (the document must not expire for at least 90 days from the date of submission).

California residents only - In addition to a current ATF-8 form, you must submit a current centralized list of firearms dealers (CFD) specific for your chosen FFL holder.

If your FFL holder will not provide a copy of the ATF-8 form, please email Fundraise@MidwayUSAFoundation.org.

We, the requesting organization, hereby agree to the following terms and conditions and have reviewed the Product Grant Application and Fulfillment Policy published on the Fundraising Products page at https://www.MidwayUSAFoundation.org/FundraisingProducts/.

  1. We will make a diligent effort to raise the fundraising goal per Product Grant item. The MidwayUSA Foundation encourages us to exceed the goal, if possible.
  2. We will contribute the minimum donation for each item(s) received to a MidwayUSA Foundation Endowment through ONE SINGLE DONATION. 
    1. For check contributions, we will utilize the Product Grant Check Donation form posted on the Fundraising Products page and include the Order Number and Item Description. We agree to use one form per Order Number. 
    2. For online contributions, we will utilize the comment section to identify the Order Number and Item Description.
  3. The MidwayUSA Foundation encourages us to complete all fundraising activities and contribute the minimum donation associated with this Product Grant and within nine (9) months of receipt.
  4. We acknowledge donations cannot be made prior to receiving an item but can be made prior to the conclusion of our fundraiser.
  5. We understand failure to designate the details of a Product Grant donation will result in MidwayUSA Foundation processing the contribution as a general donation.
  6. We understand Product Grant donations may be eligible for a match and have reviewed the Matching Program details that are published at https://www.MidwayUSAFoundation.org/Matching-Program/.
  7. We understand donating less than the minimum donation amount per item could result in losing the opportunity to participate in future MidwayUSA Foundation fundraising programs.
  8. We understand that MidwayUSA Foundation is not liable for any injuries or losses incurred by using this product, and will review the manufacturer's/dealer's literature/website for product details, proper usage, and product limitations.
  9. We understand it is our responsibility to ensure that the receiving state's firearm roster and gun laws have been checked and that the product grant can be legally received.
  10. We understand products returned to the manufacturer or distributor which we, the requesting organization or FFL holder, were unavailable to receive the shipment will result in the request being canceled and the products reallocated to other participants.
  11. We understand if there is shipping damage to the Product Grant, we, or FFL holder, will contact the Foundation within seven (7) days of the incident.
  12. We understand all fundraising products offered by MidwayUSA Foundation are provided on a first come, first served basis for as long as each Product Grant is available.
  13. We have checked our state and local regulations for raffle licensing requirements.
  14. We acknowledge all donations are unrestricted and the property of MidwayUSA Foundation, under the control of the Board of Directors, and have reviewed the donation policy at https://www.MidwayUSAFoundation.org/Policies/.
