You must be an official representative of the requesting organization and can take responsiblity of the product grant to proceed with this application.
Firearms will ship directly from the manufacturer or distributor to the FFL holder of your choice. Participants are required to submit a legible, current ATF-8 form (the document must not expire for at least 90 days from the date of submission).
California residents only - In addition to a current ATF-8 form, you must submit a current centralized list of firearms dealers (CFD) specific for your chosen FFL holder.
If your FFL holder will not provide a copy of the ATF-8 form, please email Fundraise@MidwayUSAFoundation.org.
